Authorisation Forms

At the Shire of Harvey Community Recovery Meeting held on 9 March 2016 the Project Manager for the clean-up, Mr Rene Baur, provided an update on the clean-up process and spoke about the authorisations required from property owners and tenants.

The authorisations are contained in a number of forms that can be found below with an explanatory document to assist you in completing the forms.  The forms seek approvals and important information from you regarding the clean-up of your property.

The Department of the Premier and Cabinet has considered the concerns raised at the Community Recovery Meeting regarding Clause 8 of the Authority to Act.  This Clause has been amended.  Please see the attached explanatory information for more details.

Even if your home was not damaged or destroyed it may still be appropriate to complete some of the forms, e.g. you may have a damaged shed, require temporary accommodation while clean-up work occurs nearby or you may wish to have contractors access your property for minor repairs.

The authorisation forms will need to be returned to the Department of the Premier and Cabinet either by mail (c/o State Recovery Coordinator, Locked Bag 3001 WEST PERTH  WA  6872) or scanned and sent via email to firecleanup@dpc.wa.gov.au.

Before your destroyed or damaged home or other buildings on your property can be removed you will be required to obtain a demolition licence.  The relevant forms will be or have been issued by your Shire and these need to be returned to your Shire directly.

Whilst the State Government and your local council are working hard to prepare for the clean-up of the bushfire-affected areas, your help in completing the forms and returning them as quickly as possible will assist us greatly.
